Liability Waiver and Release of Claims

By submitting this form, I, the parent or legal guardian of the participant, understand and acknowledge that participation in the Ridge Church Summer Athletics involves physical activity and carries a risk of injury. I voluntarily assume full responsibility for any risk, including injury, illness, or loss that may result from participation in camp activities, including but not limited to athletic training, games, and recreational events. I hereby release, waive, discharge, and hold harmless Ridge Church, Ridge Christian Academy, its officers, directors, employees, volunteers, and agents from any and all liability, claims, demands, actions, or causes of action arising out of or related to any loss, damage, or injury, including death, that may be sustained by the participant or any property belonging to the participant, whether caused by the negligence of the releasees or otherwise, while participating in or traveling to or from the camp. I certify that the participant is in good physical health and able to engage in athletic activities. I authorize Ridge Church / Ridge Christian Academy staff or volunteers to obtain emergency medical care as deemed necessary for the health and safety of my child, and I accept financial responsibility for such care. I have read and fully understand this release of liability and agree to its terms freely and voluntarily.
